Bodyshop Authorization for Repair

In consideration for Beaman Body Shop's repairing my vehicle, I hereby agree to the following:

1. I hereby authorize the repair work to be done with the necessary materials, and agree that Beaman will not be responsible for loss or damage to the vehicle or articles left in vehicle, in case of fire, theft, or any cause beyond their control.

2. I futher authorize Beaman, in its best and honest judgement, to either repair or replace parts on my vehicle, and I defer to such decision as they shall make even if this repair or replacement differs from insurance estimated repair. Beaman's completed repairs shall not exceed the insurance company's allowance except by agreement with them, and will not result in additional cost to me, unless I approve an additional amount which I intend to pay. I hereby grant power of attorney for the specific purpose of signing authorization forms and drafts for repair cost only.

Policies and Information

1. Due to circumstances beyond our control, delivery dates are not guaranteed, and rental responsibility is not accepted.

2. All parts prices are subject to invoice. If parts listed on the appraisal are not available, we reserve the right to repair such parts and adjust the repari accordingly.

3. Beaman Group is not responsible for any items left in the vehicle. For your protection, please be sure all your belongings have been removed.

4. If your vehicle is not picked up within 48 hours after we notify you of completion, a daily storage rate will be charged.

5. A standard hazardous waste charge of $15.00 is applied to every repair order invoice. This charge reflects the cost of legally disposing of hazardous waste material.

6. If customer has a check from the insurance company payable to them and a lein-holder, it is the customer's responsibility to have the check signed by all parties.

7. Payment of the deductible must be paid in full and payment from the insurance company must be verified upon pick-up of the insurance claim.

8. If a customer has an insurance check for damages or receives a check after repairs are completed it is the customer's responsibility to pay the body shop the amount of the payments made by the insurance company.

I understand that my vehicle will not be released until Beaman has received payment in full, and by my signature, I acknowledge that I have read and agreed to the policies stated herein:
